Overview of PDF and Word Forms

In Provet Cloud, you can upload and use fillable PDF and Word forms for various purposes, such as patient, client, consultation, discharge notes and patient referral feedback forms during consultations.

Note

It is no longer possible to create new forms using the Build form function. If you have created forms using the old form builder tool, your forms are still available and you can edit them. While Word forms are still available, it is recommended that you replace them with PDF forms.

Note

You must use a version of Microsoft Word that supports the DOCX file format. Word for web is not supported. Additionally, versions of Microsoft Word meant for Apple computers (using macOS) cannot be used.
