Client and patient forms are used to document important information about animals and their owners. These forms are used to record various details such as medical histories, treatment plans, and client communications.
To add a client or patient form to Provet Cloud:
- Find the client or patient you want to view on the Records > Clients & Patients page or click the client or patient name anywhere else it is available.
- On the client and patient page, go to the Forms tab. All forms that have been added for this client or patient are shown.
- From the Add drop-down list, select the form you want to add from a list of available forms.
- The preview dialogue opens.
- Provet Cloud autosaves the possible changes you make to the form. For other available options, see Use PDF Forms or Use Word forms.
- After closing the preview, the form is shown on the Forms tab in Provet Cloud. All saved client, and patient forms are also accessible for printing, editing, or deleting on the Forms tab.
You can also view patient forms on the History tab of the Patient page. Select the More drop-down list to filter the view to show forms.
Client forms work similarly to patient forms but are specific to the client and have no link to the patients. Unlike patient forms, client forms do not appear in the Patient History.
