Use Referral Feedback Forms

Referral feedback forms are used to communicate important information about a patient’s progress and outcomes to referring veterinarians.

To add a referral feedback form to Provet Cloud:

Scroll down to the Feedback to referring vet section at the bottom of the page and select the plus button to add a new entry. Note that the patient must be marked as a referral and have an active consultation.
Select the plus button.
Select a form from the Add form drop-down.
The preview dialogue opens.
Provet Cloud autosaves the possible changes you make to the form. For other available options, see Use PDF Forms or Use Word forms.

The added forms are shown under Forms on the Feedback to referring vet section. You can attach referral feedback forms in addition to other text and data, and email this information back to the referring vet.
